Description
Are you working on submitting your necessary health clearance documents? Use this form for any additional health immunizations or clearances still needed to be submitted to Med-Proctor.
Have you moved or changed your phone number? Update your permanent address in your home country, your address in Hawaii, or your telephone number with the address and phone number change form.
Are you changing your major? If so, submit a change of major form to update your I-20.
Do you need to add/drop a course after the add/drop deadline? Use this form to make changes to your class schedule, including changing your grading option to CR/NC.
Do you need more time to graduate? Use the Program Extension form and meet with your academic counselor to receive a revised graduation date. Then submit this form to HIC for review to see if you qualify. F-1 international students must be making normal academic progress and must not have any total number of 8 or more Fs, Ws, and/or NCs.
Qualified F-1 international students may apply for an academic RCL approval due to academic difficulty in their initial semester or graduating semester with an associates degree or certificate of achievement
Qualified F-1 international students may apply for a medical RCL with the appropriate U.S. Licensed physicianʻs approval.
Are you enrolled in an internship course or interested in learning more about internship opportunities in the future? If so, contact HIC for CPT workshop information. All F-1 international students must attend the workshop before applying for CPT.
Are you graduating this semester or interested in post-graduation employment? If so, contact HIC for OPT workshop information. All F-1 international students must attend the workshop before applying for OPT.
Have you completed a Certificate of Achievement or Associateʻs Degree and wish to reapply to Kapiʻolani CC? Complete this form and work with a HIC team member to learn more about this process.
